Policies

Cancellation
If you cannot keep your scheduled appointment, you must notify our office in writing 24 hours or more in advance of the original appointment time. Cancellations within the 24-hour notice period will be charged 100% of the session fee and may be rescheduled once. Thereafter, no refund will be given.

Consultation
Initial consultations are required. This is an opportunity for the client and provider to assess fit and build a therapeutic rapport. Should we agree to work together, you may book subsequent appointments online. Please refer to FAQs for guidance on preparation for your first appointment.

Controlled Medications
Controlled medications are NEVER prescribed on the first visit. We will take time to review records and gather collateral information from national databases prior to doing so. Clients that receive controlled medications are subject to a signed controlled substance agreement.

Crisis and/or Emergencies
If you are having thoughts of harming yourself or someone else, call 911 or report to the emergency department. This is a requirement as we cannot provide 24-hour crisis coverage.
Due to the limits of a boutique practice, we also cannot accept clients for psychiatric medication management with active suicidal or homicidal ideation, active eating disorders, active substance use disorders, or those living with chronic pain on opiate medications.

Payment
Payment for medical clients is due at time of service by cash, check, or HSA. We do not participate in Medicaid, Medicare, or Medicaid Managed Care. We are out of network for all health insurance plans. We will provide a super-bill for clients to submit to their insurance provider at their own convenience.
Payment arrangements for healthcare advocacy and corporate clients are due 7 days after date of invoice.
Coaching clients can buy a package or pay at time of service. Coaching packages are non-refundable and expire if unused 90-days from date of purchase.

Primary Care Provider (PCP) Requirement
Primary care and preventative medicine is essential for long-term physical and mental health. Due to the potential metabolic effects of psychiatric medications, clients of DeGrate + Co who receive psychiatric medication management services are required to have an active relationship with a primary care provider (PCP). An active relationship with a (PCP) means that a wellness visit has taken place within 12 calendar months.

Refill Requests
At each appointment, your provider will discuss your medications. This should eliminate the need for urgent refill requests. In the rare event an urgent refill is needed, please submit the request in writing. Refills are handled during business hours Tuesday thru Thursday. Refills are for active clients only. Controlled substances will not be refilled without an appointment within 30 days of the request.

Social Media/Personal Relationships
Clients are welcome to follow the professional social media pages of individual clinicians and/or the agency. However, it is inappropriate to attempt to “friend” or follow personal pages of DeGrate + Co clinicians or staff.
Further, clinicians with DeGrate + Co do not engage in personal (non-therapeutic) activities with clients or attend in-person social gatherings when clients are present. This is for the privacy and protection of both clients and clinicians.

Termination of Services
Successful behavioral health services are built on a strong therapeutic relationship. Unfortunately, there are circumstances when that therapeutic relationship is irretrievably broken and/or a client’s acuity exceeds the expertise of the clinician or the limits of the practice. When this occurs, the clinician will provide 14-days notice in writing and a list of resources. In rare instances, advanced notice is not possible (e.g. threats of violence, harm to self/others). In this case, a written statement will be provided with a list of resources and date of termination of services.
