What ages do you serve?
DeGrate + Co provides medical services including psychotherapy and medication management to clients 18 and older. Our non-medical services have no age restrictions.

Do you take health insurance?
DeGrate + Co is currently out of network for all insurance plans including Medicaid and Medicare. Most plans have an out-of -network benefit and will reimburse a predetermined portion for medical services to clients who submit a bill. DeGrate + Co is unable to submit bills to insurance on behalf of clients.
Payment (HSA, Credit, or Debit) is due at time of service for medical and coaching clients. Financial terms for consulting and corporate clients are detailed in contracts issued prior to the assumption of care.

Do you offer a sliding scale for any services?
We do not accept sliding scale for psychiatric medication services. We have a limited number of openings for clients needing financial support for coaching and psychotherapy. These spots are filled at this time. Please reach out to inquire about a waiting list.

What is behavioral health?
Behavior is the sum of a persons actions and interactions.
It is, quite simply, being human.
Behavioral health is the science that supports healthy actions and interactions—or, healthy humanity. We use the term behavioral health, rather than just mental health, to make the point that healthy humans require a holistic approach to care. This includes looking at elements beyond medications such as nutrition, movement, preventative medicine, and sleep hygiene.

Do you prescribe controlled substances such as benzodiazepines (Ativan, Klonopin, etc) or sleep aids (Ambien, Lunesta, etc)?
We utilize and respect all of the tools in the behavioral health toolbox. However, due to the limitations of a boutique practice, DeGrate + Co cannot accept new clients for psychiatric medication management who 1.) are current, long-term (>3 months) utilizers of benzodiazepines or sleep aids. 2.) receive non-cancer-related chronic pain regimen that includes opioids. 3.) have active suicidal/homicidal ideation . 4.) have an active eating disorder (<12 months remission) , or 5.) have an active substance use disorder (<12 months remission).

How do I prepare for a psychiatric assessment?
The purpose of an initial psychiatric evaluation is to gather a full clinical portrait. Below are helpful action items to prepare:
•Please complete paperwork in full and submit 24 hours prior to appt.
•Have a record of current and past medications, and allergies.
•Record a weight, blood pressure, and heart rate the morning of the appt.
•List of current physicians, their specialty, and dates of service
•Family medical and mental health history

What is the difference between coaching and counseling?
Coaching is short term, non-medical, goal-oriented guidance that examines how your present situation impacts your future. Coaching clients benefit from structure and accountability on their way to an uncharted territory. Coaching is not usually reimbursed by insurance plans.
Psychotherapy is evidence-based treatment that examines the past and its influence on your present, to build a better future. Psychotherapy can be focused, middle, or long-term depending on the client and modality (approach). It is often reimbursed by insurance plans.

What is Nurse Navigation?
Nurse navigation is care management by a registered nurse for clients or families facing physical or mental illness. After a comprehensive evaluation, clients typically receive support reviewing medical records, setting up appointments, and coordinating care. Nurse navigators also serve as an easily accessible resource to ask questions. Nurse navigators do not replace your care team. They work alongside your care team to alleviate the administrative pressures of illness, leaving you to focus on wellness.

What is an "active client"?
Clinicians with DeGrate + Co can only provide services such as refill requests and crisis interventions to active clients. Active clients are those who have had an initial psychiatric intake and at least 1 follow up within the past 90 calendar days. Clients who seek out psychiatric medication management from a different physician or advanced practice provider at any time after their initial intake forfeit their active status with DeGrate + Co and must be readmitted to the practice.

What is palliative care?
Palliative care is a specialty that focuses on optimizing comfort throughout the life of a healthcare consumer. It is not just for those with terminal illness. Palliative care emphasizes symptom management with great respect toward the whole person—body and mind.

How does palliative care differ from hospice care?
Hospice care is for clients reaching the end of their life. To receive hospice care, two physicians must certify that a person has 6 months or less to live. Palliative care is comfort and symptom management for clients regardless of their prognosis.
Simply put: All hospice patients receive palliative care, but not all palliative care patients receive hospice.

Some of our clients find us when they’ve tried for so long to manage their illness themselves and are exhausted. Others, seek us out at the very beginning of a new diagnosis and want help for what is to come. Still others engage our firm while they are still relatively healthy and just want to ease the way for themselves or their loved ones.
